These four documents comprise the 2008 Custer's Last Stand FoodFest Application. They are in PDF format and may be downloaded and printed.

The Food Fest document contains all necessary information regarding process and pricing. Mail the "Mail-Back Form", the "Clean-Up Form", a copy of the "City of Evanston Application" and the Insurance Certificate to:

Include full payment with your application. The "City of Evanston Application" should be filled out and mailed back along with their fee to the City of Evanston per their instructions.
